Job Summary
Minimum Qualifications and Experience:
- B. Pharm and/or MSc (Med) Pharm or M. Pharm (Clinical Pharmacy)
- 5–7 years of experience in a clinical pharmacy setting
- Hospital experience is essential
- Proven operational experience and application of clinical pharmacy expertise in a hospital environment (non-negotiable)

Requirements:
- Lead designated patient service lines and therapeutic programs.
- Collaborate with the Group Clinical Pharmacist to implement medication utilization frameworks, standards, and systems across hospitals.
- Oversee pharmaceutical care services, direct patient care programs, medication utilization systems, and care areas to ensure alignment with patient care needs, best practices, and regulatory requirements.
- Review and monitor patient profiles/charts to identify, prevent, or resolve drug-related issues, inappropriate medication use, dose selection errors, over-prescription, medication without indication, and treatment failures.
- Provide pharmaceutical services across the entire hospital facility.
- Ensure all transactions are properly recorded and reported on the approved platforms and systems.
- Deliver accurate, safe, and timely medication therapy tailored to individual patient needs.
- Ensure recommendations account for cost-effective initiatives adopted within the group.
