Durban: Case Manager (Hospital / Inpatient Programs)

About the Role

We are seeking a skilled Case Manager to join our team in Durban, working with patients in hospital/inpatient programs. The ideal candidate will have extensive experience in inpatient mental health settings and possess strong communication skills.

Key Responsibilities

  • Conduct intake assessments and verify referral information
  • Develop and adjust individualized treatment plans
  • Coordinate psychiatry, psychology, nursing, OT, and social work teams
  • Facilitate MDT meetings and track patient progress
  • Liaise with families, employers, and referring practitioners (with consent)
  • Lead discharge planning and aftercare coordination
  • Monitor clinical risks and advocate for patients
  • Maintain accurate, confidential records in compliance with MHCA, POPIA, and professional standards

Requirements

  • Registered Nurse (SANC), Social Worker (SACSSP), or Occupational Therapist / Psychologist / Registered Counsellor (HPCSA) with active registration
  • Minimum 4 years experience in inpatient mental health, psychiatric, or rehabilitation settings
  • Knowledge of multidisciplinary treatment models and mental health legislation
