Job Summary

Hire Resolve is looking for an experienced and dedicated Case Manager to join a reputable healthcare facility in Mitchells Plain. In this role, you will be responsible for coordinating and managing patient care services and ensuring that patients receive appropriate, high-quality care tailored to their individual needs. Your expertise will be essential in developing care plans, collaborating with healthcare providers, and providing support to patients and their families.

Responsibilities:

  • Conduct comprehensive assessments of patients medical, physical, and psychosocial needs.
  • Develop, implement, and evaluate individualized care plans in collaboration with the healthcare team.
  • Coordinate patient care services across various settings including hospitals, outpatient services, and home care.
  • Serve as a liaison between patients, families, and healthcare providers to ensure effective communication and care continuity.
  • Monitor patient progress and adjust care plans as necessary based on evolving needs.
  • Provide education and resources to patients and families on health management and available services.
  • Maintain accurate and up-to-date patient documentation and case management records.
  • Advocate for patients to ensure they receive necessary services and benefits.
  • Participate in interdisciplinary team meetings to optimize patient care.
  • Stay current with changes in healthcare policies, procedures, and best practices.

Requirements

  • Bachelor`s degree in Nursing, Social Work, or a related healthcare field.
  • Minimum of 2 years of experience in case management or a similar role, preferably in a healthcare setting.
  • Strong understanding of healthcare systems and community resources.
  • Excellent communication, interpersonal, and organizational skills.
  • Ability to work independently and collaboratively as part of a healthcare team.
  • Strong critical thinking and problem-solving abilities.
  • Valid drivers license and willingness to travel as needed.
  • Certification in case management (CCM) is a plus.
  • Knowledge of electronic health records (EHR) and case management software.
