About the Role
Faircape Health is extending our subacute care and rehabilitation services to one of our long-term healthcare facilities in Somerset West, Heritage Manor. We are catering to patients in need of short-term, long-term, dementia, respite, and palliative care within a secure and nurturing environment.
Key Responsibilities
- Conduct comprehensive assessments of patients’ medical conditions
- Develop and implement treatment plans tailored to each patient’s specific needs
- Collaborate with a multidisciplinary team of healthcare professionals to coordinate patient care
- Monitor patients’ progress closely and make adjustments as necessary
- Provide patients and their families with information about their medical conditions and treatment options
- Maintain accurate and up-to-date medical records
- Participate in quality improvement initiatives
Requirements
- Proven post-community service experience in internal medicine, family medicine, geriatrics, or palliative care is beneficial
- Experience working within a multidisciplinary team environment advantageous
- Proficiency in diagnosing and treating various medical conditions with strong understanding of evidence-based practices
- Good Critical Thinking Skills
- Ethical Practice and confidentiality
- Strong Team Collaboration ability
- Meticulous attention to detail in documentation
- Strong Communication Skills

- Bachelor of Medicine & Bachelor of Surgery (MBChB)
