Job Description
Key Responsibilities
- Conduct intake assessments and verify referral information
- Develop and adjust individualized treatment plans
- Coordinate psychiatry, psychology, nursing, OT, and social work teams
- Facilitate MDT meetings and track patient progress
- Liaise with families, employers, and referring practitioners (with consent)
- Lead discharge planning and aftercare coordination
- Monitor clinical risks and advocate for patients
- Maintain accurate, confidential records in compliance with MHCA, POPIA, and professional standards
Minimum Requirements
- Registered Nurse (SANC), Social Worker (SACSSP), or Occupational Therapist / Psychologist / Registered Counsellor (HPCSA) with active registration
- Minimum 4 years’ experience in inpatient mental health, psychiatric, or rehabilitation settings
- Knowledge of multidisciplinary treatment models and mental health legislation
- High emotional intelligence and strong communication skills
